Inframammary Breast Augmentation This 25-year-old woman from the McAllen, Texas area presented to my cosmetic surgery clinic requesting breast augmentation. She is 5’3” tall, and weighed 134 pounds. She wanted very full breasts and liked the Ultra High Profile silicone gel implants. We performed an inframammary breast augmentation with Allergan Natrelle Inspira 650cc Ultra High Profile silicone gel implants placed in the sub-pectoral position (dual-plane under the muscle). The challenge for her breast augmentation was to release her tight inframammary folds and correct her Tubular Breasts. We discussed that it would take a minimum of 4 months for her tissues to properly stretch and remodel around her implants. For her breast augmentation, we used Mentor 450cc Moderate Plus Profile Saline implants placed in the subpectoral (dual plane) position and overfilled to 475cc bilaterally. We released her restricted inframammary folds and performed radial cuts of the dense tubular breast tissue to facilitate a more natural re-draping of the tissue over her implants. She was a 38B and is now a 38D. Her results are shown at 4 months, and she loves her cosmetic surgery results.

The challenge for her breast augmentation was to release her tight inframammary folds and correct her Tubular Breasts. We discussed that it would take a minimum of 4 months for her tissues to properly stretch and remodel around her implants. For her breast augmentation, we used Mentor 450cc Moderate Plus Profile Saline implants placed in the subpectoral (dual plane) position and overfilled to 475cc bilaterally. We released her restricted inframammary folds and performed radial cuts of the dense tubular breast tissue to facilitate a more natural re-draping of the tissue over her implants. She was a 38B and is now a 38D. Her results are shown at 4 months, and she loves her cosmetic surgery results.

TransAxillary Breast Augmentation

This challenging case demonstrates what can be accomplished with a transaxillary breast augmentation. These results required not only careful surgical planning and execution, but demanded that the patient to be compliant with her post-operative instructions over 4 months. Quite honestly, even I was amazed with the final result! This 28 year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting breast augmentation. She is 5’3” tall and weighed 127 pounds. She was interested in saline implants and she wanted to avoid any scars on her breasts. We discussed the challenge that her anatomy presented for either a transaxillary or a transumbilical approach. She was very muscular with extremely tight inframammary folds. She also had chest wall asymmetry with her left chest wider than the right. We agreed to proceed with a transaxillary approach with the understanding that her implants would take 3 – 6 months to settle, and that a second stage inframammary revision may be necessary. I performed the transaxillary breast augmentation, and placed Mentor Moderate Plus Profile saline implants in the subpectoral position. On the left, a 375cc implant was filled to 425cc, and on the right a 350cc implant was filled to 385cc. She was very compliant with her skin redraping exercises and implant massages post-op, and her results are shown at 4 months. Her implants settled beautifully and no revision was necessary. She was a 34A and is now a 34D, and she loves her cosmetic surgery outcome.

She selected the inframammary approach and silicone gel implants. During her consultation we discussed her breast size asymmetry (her left breast measured approximately 25ml larger than the right) and her Grade I breast ptosis (droop). We discussed the use of two different sized implants to improve this asymmetry as best as possible and that she would not require a formal breast lift. The patient preferred the moderate plus profile implants versus the high profile implants and she selected implants that were appropriate for her body size and chest width. We performed her breast augmentation using Mentor Moderate Plus Profile silicone gel implants in a dual-plane subpectoral position, and we were able to conceal her small incisions very nicely in her inframammary fold. We placed a 400cc implant on the left and a 425cc implant on the right. Her implants settled very nicely, and both her breast size and nipple-areolar asymmetries were nicely corrected.
