CASE 722: Complex Repair of Symmastia
Age: 26 | Height: 5’9″ | Weight: 140

This 26 year-old woman from the McAllen, Texas area presented to my plastic surgery clinic seeking help to correct her symmastia. She is 5’9” and weighs 140 pounds. She had undergone prior inframammary subpectoral breast augmentation with Allergan 375cc high profile silicone gel implants by another board-certified general plastic surgeon. Her procedure was complicated by symmastia (over-dissection medially while dissecting the implant pocket and releasing the sternal attachments of the pectoralis muscle). The pre-operative photo above shows the symmastia (sometimes called “uniboob”) with kissing implants. This patient had undergone 5 prior failed repair attempts by her 1st surgeon. She now sought definitive repair. We discussed the complexity of symmastia repair. Historically, some surgeons would remove the subpectoral implants for 3 months, and then re-operate to place new implants in the subglandular position. This patient did not wish to have her implants removed or have additional staged procedures. We successfully performed a complex, single-stage symmastia repair with internal restructuring (capsulorrhaphy) reinforced with absorbable Vicryl™ mesh. We performed this procedure minimally invasively through her small, pre-existing inframammary scar using an advanced parachute technique. A scar revision was also performed. The patient was very compliant with her post-op instructions and use of the special bra. Her successful results are shown 3 months after her surgery, and she is extremely happy with her cosmetic surgery results!

Complex Revision Breast Augmentation with Seri® Surgical Scaffold and
Bilateral Peri-Areolar Scar Revisions with Left Breast Crescent Lift

This 32 year-old woman from the Edinburg, Texas area presented to my cosmetic plastic surgery clinic requesting revision breast augmentation. She is 4’11” and weighed 114 pounds. Three years before, she had undergone breast augmentation via the peri-areolar approach and had selected 375cc Mentor moderate plus saline implants, which were placed in the subpectoral dual-plane position and filled to 385cc’s. She had been very happy with her results, but now desired larger breasts with increased upper breast fullness and cleavage. She wanted to lift her implants higher on her chest, and during her consultation I also noticed that her implants had become slightly laterally displaced due to stretching of her thin tissues.

Due to her thin body habitus and delicate tissues, we discussed the fact that simple capsulorrhaphy (internal restructuring) to lift her implant pockets and bring in her implants would likely not support her new, larger implants without addition support using SERI Surgical Scaffold.

We performed a complex revision breast surgery that included: removal of her old moderate plus saline implants; extensive lateral and inferior capsulorrhaphy repairs to correct her lateral displacement and bottomed out implants using Seri® Surgical Scaffold to re-enforce these repairs and provide additional coverage for the implants. New, larger Natrelle (Allergan) INSPIRA High Profile 560ml silicone gel implants were placed. Bilateral periareolar scar revisions were performed as part of our closure, and we also performed a crescent lift on the left to further enhance her results and symmetry. She was very compliant with her post-operative instructions and wore her supportive underwire bra 24/7 for the first 6 weeks.

She was a 34D and is now a 32DD. Her early post-op results are shown only 2 months after her surgery, and she loves her new breasts!

Complex Revision Breast Augmentation with Seri® Surgical Scaffold and Extended LeJour Breast Lift

This 36 year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting revision breast augmentation. She is 5’2” and weighs 123 pounds. Her first breast augmentation had been performed several years ago by a board-certified general plastic surgeon in another U.S. city using saline implants placed under the muscle via an inframmamary approach. She states her implants had always felt too high and too far apart. Over time, her implants had fallen further to the sides (lateral migration), most noticeably when she lies flat (see photo). In addition, she suffered progressive sagging (ptosis) of her breast skin over the implants as a result intervening pregnancies. She wanted pretty, perky breasts, located properly on her chest, that were slightly larger and softer.

We discussed the complexity of her case and the multiple problems that needed to be corrected. We needed to lower her implants and recreate her inframmamary fold, correct the lateral displacement of her implants, enhance her cleavage, re-enforce these repairs, and correct her breast ptosis (drooping) with a breast lift (mastopexy). She understood that this was not a simple breast augmentation surgery.

We performed a complex breast revision procedure. I removed her old 420ml saline implants, lowered her implant pocket (capsulotomy), and performed lateral internal restructuring (capsulorrhaphy) with medial capsulotomies to correct the lateral displacement and enhance her cleavage. I used Seri® Surgical Scaffold to re-enforce her new inframammary fold and my lateral capsulorrhaphy repairs to support her new, slightly larger Mentor High Profile 475ml silicone gel implants and prevent them from bottoming out or recurrent lateral displacement. An extended vertical LeJour mastopexy (breast lift) was performed to correct her breast ptosis. She was very compliant with her post-operative instructions and the supportive underwire bra 24/7 for the first 6 weeks.

She was a 32D and is now a 32DD. Her early post-op results are shown only 2 months after her surgery, and she already loves her new breasts and her cosmetic surgery results!

Complex Revision Breast Augmentation after removal of infected implants

This 43 year-old woman from the Mission, Texas area presented to my cosmetic plastic surgery clinic with infected breast implants that had been placed in Mexico. She is 5’6” and weighed 197 pounds. Her before images demonstrate a classic Snoopy Nose deformity where the inexperienced surgeon in Mexico simply placed implants without performing the needed breast lift. This patient had also undergone a tummy tuck procedure in Mexico, which we also had to revise.

For her breasts, we first removed the infected breast implants and allowed her body to heal. She did not elected to proceed with her redo breast augmentation with breast lift until 2 years later. At that time, we performed an extended vertical LeJour mastopexy (breast lift) with subpectoral silicone gel implants. We used 375cc Mentor moderate plus profile implants. In addition, we extended the inframammary component of her breast lift out to the side to incorporate a side body lift to excise all of the excess skin and fat from the sides of her chest. This case illustrates how important it is for patients to carefully consider who performs their operation and where their surgery is performed! This woman not only suffered a poor cosmetic result that required revision, her life was jeopardized by the surgical infection of her implants in Mexico.

She was a 38B and is now a 38D. Her post-op results are shown only 3 months after her surgery. She is very pleased and loves her cosmetic surgery results.

CASE 737: Complex Revision Breast Augmentation / Corrective Surgery with Partial Capsulectomy, Symmastia Repair, and Breast Lift

Age: 43 | Height: 5’3″ | Weight: 106

This 43 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic seeking revision breast augmentation. She is 5’3″ and weighs 106 pounds. She had undergone prior inframammary subpectoral breast augmentation in Mexico. Her procedure was complicated by residual breast ptosis (Snoopy nose deformity), capsular contracture, and symmastia (uni-boob caused by over-dissection medially with release of the pectoralis muscle sternal attachments). The pre-operative photo above shows the symmastia with kissing implants. We performed a complex revision breast augmentation procedure to correct her capsular contracture with partial capsulectomy, single-stage symmastia repair with internal restructuring (capsulorrhaphy) reinforced with Seri Surgical Scaffold absorbable mesh, and a peri-areolar mastopexy (Donut Lift). We replaced her old implants with new 330cc Mentor High Profile saline implants filled to 335cc bilaterally. The patient was very compliant with her post-op instructions and use of the special bra. Her successful results are shown 3 months after her surgery, and she is extremely happy with her results!

CASE 716: Complex Revision Breast Augmentation / Corrective Surgery with Internal Restructuring and Re-do Extended LeJour Breast Lift
Age: 63 | Height: 5’1″ | Weight: 155

This 63 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic requesting redo breast lift and revision breast augmentation. She is 5’1″ and weighs 155 pounds. She is a post-bariatric patient with a history of significant weight loss. She had previously undergone a tummy tuck and breast augmentation with a breast lift at an outside center. Her implants had been placed in the subpectoral position and were too far to the sides. She was also very unhappy with the uneven nipple positions from her previous breast lift. She wanted her breasts to be “perkier,” with enhanced cleavage, and more properly centered on her chest wall to eliminate the lateral displacement of her implants and to appear more youthful. She also wanted to have her areolas more properly centered on her breasts.

We performed a complex revision breast augmentation surgery with removal of her old silicone implants and placement of new, slightly larger Mentor Moderate Plus Profile saline implants. Two different sized implants were needed: on the Left, a 375cc implant was overfilled to 400cc; and on the Right, a 400cc implant was filled to 440cc. Additional internal restructuring (capsulhoraphy) was performed to properly center the implants on her chest wall. We also performed a redo extended LeJour breast lift (vertical mastopexy) to correct her residual ptosis and asymmetry. As part of her extended LeJour breast lift, we extended the inframammary scars around her upper back to perform an upper back lift to eliminate the excess skin from her back and sides (See Post-Bariatric Before and After photos). She was a 38C and is now a 36D. Her results are shown 2 months post-op and she loves her new look after her cosmetic surgery.

Complex Revision Breast Augmentation with SERI® Surgical Scaffold and Redo Extended LeJour Breast Lift

This 44 year-old woman from the Corpus Christi, Texas area presented to my plastic surgery clinic requesting revision breast augmentation. She is 5’1” and weighs 150 pounds. Her first breast augmentation had been performed several years ago by a board-certified general plastic surgeon in another U.S. city using textured saline implants placed under the muscle with a breast lift procedure. She states her implants had always felt a little too small and too far apart, and she was unhappy with the widening of her inframammary scar and her uneven areolas. Over time, her implants had fallen further to the sides (lateral migration), most noticeably when she lies flat (see photo). In addition, she suffered progressive sagging (ptosis) of her breast skin over the implants, and her breasts had become harder. She wanted larger, softer, perkier breasts, located properly on her chest, with smaller, more symmetrical areolas, and finer scars.

We discussed the complexity of her case and the multiple problems that needed to be corrected. Examination revealed that the patient had developed Grade 3 capsular contractures. We needed to partially excise the old, hardened capsules, correct the lateral displacement of her implants, enhance her cleavage, remove the old implants and replace with new, larger implants, re-enforce these repairs; and correct her areolar asymmetry, widened scars, and recurrent breast ptosis (drooping) with a redo breast lift (mastopexy). She understood that this was not a simple breast augmentation surgery.

We performed a complex breast revision procedure. I removed her old textured saline implants. These were 360cc implants that had been filled to 375cc on the left and 365cc on the right. I performed partial capsulectomies to remove the hardened Grade 3 capsules, and then lateral internal restructuring (capsulorrhaphy) with medial capsulotomies to correct the lateral displacement and enhance her cleavage. I used SERI® Surgical Scaffold (Allergan, Inc.) to re-enforce my lateral capsulorrhaphy repairs, support her new, larger Mentor High Profile 550cc silicone gel implants, and prevent recurrent lateral displacement. An redo extended vertical LeJour mastopexy (breast lift) was performed to correct her breast ptosis, enhance her areolar symmetry, and revise her old scars. She was very compliant with her post-operative instructions and the supportive underwire bra 24/7 for the first 6 weeks.

She was a 36D and is now a 36DD. Her post surgical results are shown at only 2 months, and she already loves her new breasts and her cosmetic surgery results!

CASE 101: Breast Enhancement After Prior Breast Augmentation
This woman from the McAllen, Texas area presented to my plastic surgery clinic requesting a breast revision. She had recently undergone breast augmentation surgery by another surgeon in the area, and she was very upset with the residual breast ptosis (drooping) and the resultant “Snoopy Nose” deformity. We performed a minimal scar LeJour Breast Lift and she is very pleased with her cosmetic surgery results.

Complex Re-Augmentation with Partial Capsulectomy, Correction of far apart breasts, and Recreation of Inframammary fold with Seri Surgical Scaffold:

This 40 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic seeking breast re-augmentation. She is 5’0” and weighs 123 pounds. She had undergone prior inframammary subpectoral breast augmentation in Mexico many years prior. Over time, her implants had become laterally displaced (separated) and she developed capsular contracture (hardening of the breast) on the right. In addition, her slender figure resulted in thin soft tissue coverage of her implants inferiorly, and she had palpable rippling. The patient desired softer breasts that were slightly fuller with increased cleavage. She didn’t know what size implants had been placed. We planned our implant selection based on her chest width diameter and her preference for High Profile implants. I performed a minimally invasive complex re-augmention procedure via her pre-existing inframammary scar using my Rodriguez Parachute Technique. A partial capsulectomy was performed to correct her capsular contracture to soften her breasts, and I corrected her breasts being apart (lateral displacement) by using complex techniques to recreate the proper place to put the implant (internal restructuring; capsulorrhaphy) reinforced with Seri Surgical Scaffold (absorbable support material) to make sure the results last over years. This gave her the cleavage she was looking for. The Seri Surgical Scaffold also allowed me to recreate her inframammary fold and addressed her thin lower breast soft tissue coverage. The Seri material adds an extra layer to support the larger implants and prevent the palpable rippling. We replaced her old implants with new 425cc Mentor High Profile silicone gel implants. The patient was very compliant with her post-op instructions and use of the special bra. Her successful results are shown only 1 month after her surgery, and she is extremely happy with her cosmetic surgery results!

This 39 year-old woman from the Mission, Texas area presented to my cosmetic plastic surgery clinic requesting a redo breast lift. She is 5’7” tall and weighed 155 pounds. She had recently undergone a breast reduction by a general plastic surgeon in the area, and she was very unhappy with her results. She disliked the dog-ears on her incisions (evident at the bottom side of her before ¾ and profile images) and her residual breast size asymmetry with the left breast larger than the right. In addition, she had wanted to preserve her upper breast fullness and was unhappy with how small her breasts were made. In order to achieve her desired outcomes and breast profile, we performed a complex redo breast lift with different sized implants and a small breast reduction on the left to optimize her symmetry. We placed Allergan INSPIRA High Profile Silicone Gel implants in the subpectoral (dual-plane) position: 345cc on the left and 365cc on the right. Her new breasts are shown at 2 months, and she loves her cosmetic surgery results!

Complex Revision Breast Augmentation with Seri® Surgical Scaffold

This 51 year-old woman from the Harlingen, Texas area presented to my plastic surgery clinic requesting revision breast augmentation. She is 5’2” and weighed 112 pounds. Several years prior, she had undergone previous breast augmentation in the U.S. by a general plastic surgeon. She had never been happy with the initial results and had always desired enhanced upper breast fullness and cleavage with perkier breasts. This patient was very slender and active, she was very displeased that she could see the demarcation from where her pectoralis muscle had been released, and over time her implants had sagged significantly. She had visible and palpable rippling, with Grade 2 breast pseudoptosis (bottoming out of the impants), as well as severe lateral displacement of the implants (made worse when laying on her back). In addition, her periareolar scars were very widened.

We performed a complex revision breast surgery that included: removal of her old 475ml silicone implants; extensive lateral and inferior capsulorrhaphy repairs (internal restructuring) to correct her lateral displacement and bottomed out implants using Seri® Surgical Scaffold to re-enforce these repairs and provide additional coverage for the implant in the lower plane of the dual-plane subpectoral pocket, which was only thin skin in this slender patient. In addition, the Seri® Surgical Scaffold will provide long-term results and prevent dropping of the implants by supporting her implants as the material is slowly reabsorbed over 24 months and replaced with native supportive tissue. New, larger Mentor High Profile 650ml silicone gel implants were placed, and a scar revision completed the procedure. She was very compliant with her post-operative instructions and the supportive underwire bra 24/7 for the first 6 weeks.

She was a 32D and is now a 32DDD. Her early post-op results are shown only 2 months after her surgery, and she already loves her cosmetic surgery results and her new look. She states that she never imagined she would have such dramatic cleavage and upper breast fullness!

Complex Revision Breast Augmentation with Seri® Surgical Scaffold and Redo Extended LeJour Breast Lift

This 40 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic requesting revision breast augmentation. She is 5’3” and weighs 141 pounds. Several years prior, she had undergone previous breast augmentation with a breast lift in Mexico. Her implants had been placed in the subglandular position (above the muscle). She had never been fully happy with the initial results. She had desired slightly smaller breasts, but with enhanced upper breast fullness and cleavage, and perkier breasts. Over time, her implants had also sagged significantly, and she had Grade 3 breast pseudoptosis (drooping of the impants). A few months prior to seeing me for consultation, she had returned to her surgeon in Mexico for a second procedure for smaller implants. She was upset that she still had the droopy breasts and she noticed that soon after her second surgery that one breast looked very different from the other.

During our consultation it was evident that her prior surgeon had simply replaced her old, larger implants with new, smaller implants, that he did not perform internal restructuring (capsulorrhaphy) to make her implant pockets smaller, and that he did not correct her droopy breasts with a redo breast lift. It also appeared on examination that her implant on the left was flipped upside down.

We discussed and performed a complex revision breast augmentation with a redo breast lift. We removed her old subglandular 460ml textured silicone implants, and intra-operative findings confirmed that her old implant on the left was flipped upside down. We placed new, slightly larger Mentor High Profile 475ml silicone gel implants in a dual-plane subpectoral position (under the muscle) using Seri® Surgical Scaffold to recreate her inframammary fold and effectively support the implants in the subpectoral position. A redo extended vertical LeJour mastopexy (breast lift) was then performed to remove the excess skin that resulted from replacing the old subglandular implant into the subpectoral position, and to correct her droopy breasts. This redo breast lift also served as a scar revision for this patient. She was very compliant with her post-operative instructions and the supportive underwire bra 24/7 for the first 6 weeks.

She was a 36D and is still a 36D, but with increased upper breast fullness, nicer cleavage, perkier breasts, and finer scars. Her early post-op results are shown only 2 months after her surgery, and she already loves her cosmetic surgery results and her new look.

Complex Revision Breast Augmentation with Seri® Surgical Scaffold and Extended LeJour Breast Lift

This 34 year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting revision breast augmentation. She is 5’8” and weighs 158 pounds. Several years prior, she had undergone previous breast augmentation in Mexico. Her implants had been placed in the subglandular position (above the muscle). She had never been happy with the initial results and had always desired enhanced upper breast fullness and cleavage with perkier breasts. In addition, her nipples and areolas we very laterally displaced naturally, and she desired these to be more centrally located on her breast. Over time, her implants had sagged significantly, she had visible and palpable rippling, and she had Grade 3 breast pseudoptosis (drooping of the impants) as well as severe lateral displacement of the implants (side boobs into her armpits).

We performed a complex revision breast surgery that included: removal of her old subglandular 400ml textured silicone implants; placement of new, larger Mentor High Profile 650ml silicone gel implants in a dual-plane subpectoral position (under the muscle) using Seri® Surgical Scaffold to recreate her inframammary fold, correct her lateral displacement, and effectively support the implants in the subpectoral position. An extended vertical LeJour mastopexy (breast lift) was also performed to remove the excess skin that resulted from replacing the old subglandular implant into the subpectoral position, and to correct the lateral displacement of her areolas. This 31 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic for breast enhancement. She is 5’2″ and weighs 107 pounds. She had undergone previous breast augmentation at an outside center. Her implants had been placed in the subglandular position, and overtime her implants had bottomed out and became laterally displaced, her skin became stretched out, and there was visible rippling of the implants.

She desired to restore her ‘perky’ breasts and make her breasts look better. We accomplished this with a corrective revision breast augmentation surgery. We removed her old implants, placed new Mentor Silicone 425cc High Profile implants in the subpectoral position, and performed a peri-areolar mastopexy (Donut Lift). She was a 34D and is now a 34D. Her results are shown 3 months post-op and she is thrilled with her cosmetic surgery results.

This 33 year-old woman from the Edinburg, Texas area presented to my plastic surgery clinic for revision breast augmentation. She is 5’4″ tall and weighed 116 pounds. She had undergone prior breast augmentation with saline implants and a peri-areolar (Donut) lift by another surgeon several years ago. Her primary complaints were that her areolas had become widened, she disliked the rippling of the saline implants, and she desired to be slightly bigger with enhanced cleavage. The patient did recall that her other surgeon had used two different sized implants to address her breast size asymmetry. Performed a revision breast augmentation with larger Mentor High Profile silicone gel implants. After removing her old saline implants, we selected a 475cc implant for the left and a 450cc implant for the right. We then revised and redid the peri-areolar (Donut) breast lift to make her areolas smaller and more symmetrical. She was a 34C and is now a 34D. Her results are shown at only 2 months post-op and she loves her new look after her cosmetic surgery.

We performed a complex revision breast augmentation surgery with removal of her old subglandular 495ml silicone implants and placement of new, slightly smaller Mentor High Profile 450ml silicone gel implants in the subpectoral position (under the muscle). We accomplished with this with reconstruction of her inframammary fold and lateral chest wall soft-tissues and internal restructuring. We also performed a redo extended LeJour breast lift. She was a 36DDD and is now a 38D. Her results are shown 2 months after her surgery post-op and she is thrilled with her cosmetic surgery results.

CASE 198: Complex Revision Breast Augmentation / Corrective Surgery with Internal Restructuring and Donut Lift

CASE 553: Complex Revision Breast Augmentation / Corrective Surgery with Internal Restructuring and Extended LeJour Lift

Overtime her implants had bottomed out and her skin had become overly stretched. She desired to restore her ‘perky’ breasts and upper breast fullness. We accomplished this with a complex revision breast augmentation surgery with subpectoral implant exchange, internal restructuring, and an extended LeJour Lift. We used 450cc Mentor Moderate Plus Profile saline implants overfilled to 530cc on the left and 525cc on the right. She was a 34DD and is now a 32DD.
