Fleur-de-Lys Abdominoplasty & Upper Body Lift
This 33-year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting post-bariatric body contouring surgery. She had undergone a laparoscopic gastric sleeve procedure 2 years before, and had lost over 200 pounds! She was 360 pounds and now weighed 158. Her weight had been stable for the past 6 months. She is 5’5”. Her primary concern was the excess skin and fat that remained around her abdomen, torso, and arms. She had very little excess fat or skin around her outer thighs and buttock, but she had severe excess skin throughout her upper and mid abdomen. We performed a Fleur-de-Lys Abdominoplasty to fully excise all of her excess skin, with full plication of her diastasis recti. The patient understood the associated scarring with this extended Fleur-de-Lys (French Flower) pattern tummy tuck, and understood why this was necessary. We also discussed a second staged upper body lift to address her excess skin and tissue around her upper abdomen, sides, and back that could not be corrected with the Fleur-de-Lys abdominoplasty. Her results are shown first at only 2 months after the first procedure, and then one-year later after her upper abdominal lift. Her case illustrates the scar maturation process over 1 year and she had a very beautiful result with an almost invisible scar! Her weight is down to 145 pounds. Extended Arm Lift

She had undergone a laparoscopic gastric sleeve procedure 2 years before, and had lost over 200 pounds! She was 360 pounds and now weighed 158. Her weight had been stable for the past 6 months. She is 5’5”. Her primary concern was the excess skin and fat that remained around her abdomen, torso, and arms. We first performed a Fleur-de-Lys Abdominoplasty for this patient (See Tummy Tuck and Post-Bariatric photo galleries for more details). One month after her tummy tuck, we performed the extended arm lift as a staged procedure. For this patient, we extended the arm lift excision down the side of her chest to address her concerns from excess skin in this area as well, and performed a modified side body lift. Her post-op arm lift results are now shown 1 year post-op. She has healed beautifully and is very pleased by her scarring. She loves her cosmetic surgery results!

CASE 716: Upper Back Lift with Complex Revision Breast Augmentation / Corrective Surgery with Internal Restructuring and Re-do Extended LeJour Breast Lift
Age: 63 | Height: 5’1″ | Weight: 155

This 63 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic requesting a redo breast lift and revision breast augmentation and an upper back lift. She is 5’1″ and weighs 155 pounds. She is a post-bariatric patient with a history of significant weight loss. She had previously undergone a tummy tuck and breast augmentation with a breast lift at an outside center. Her implants had been placed in the subpectoral position and were too far to the sides. She was also very unhappy with the uneven nipple positions from her previous breast lift. She wanted her breasts to be “perkier,” with enhanced cleavage, and more properly centered on her chest wall to eliminate the lateral displacement of her implants and to appear more youthful. She also wanted to have her areolas more properly centered on her breasts.

We performed a complex revision breast augmentation surgery with removal of her old silicone implants and placement of new, slightly larger Mentor Moderate Plus Profile saline implants. Two different sized implants were needed: on the Left, a 375cc implant was overfilled to 400cc; and on the Right, a 400cc implant was filled to 440cc. Additional internal restructuring (capsulhoraphy) was performed to properly center the implants on her chest wall. We also performed a redo extended LeJour breast lift (vertical mastopexy) to correct her residual ptosis and asymmetry. As part of her extended LeJour breast lift, we extended the inframammary scars around her upper back to perform an upper back lift to eliminate the excess skin from her back and sides. She was a 38C and is now a 36D; her weight is now down to 140. Her results are shown only 2 months post-op and she is thrilled with her cosmetic surgery results.

This 44-year-old woman from the McAllen, Texas area presented to my cosmetic plastic surgery clinic requesting post-bariatric body contouring surgery. She had lost 128 pounds over 21 months with diet and exercise. Her weight had been stable for the past 6 months. She is 5’1” and now weighed 146 pounds. Around-the-World Abdominoplasty
Extenteded Tummy Tuck with Posterior Body Lift, and additional liposuction to the outer thighs with VASER liposuction flanks and back

This 52-year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting post-bariatric body contouring surgery. She had undergone a laparoscopic gastric sleeve procedure 5 years before, and had lost 70 pounds. She was 275 pounds and now weighed 205. She is 5’3”. Her primary concern was the excess skin and fat that remained around her abdomen, torso, outer thighs, and buttocks. We discussed the Around-the-World Abdominoplasty, which combines an extended abdominoplasty (tummy tuck) with full plication of her diastasis recti, and a posterior lower body lift (butt lift). The posterior lower body lift also includes an outer thigh lift with liposuction to the outer thighs. For her abdominoplasty, we did discuss a possible Fleur-de-Lys Abdominoplasty to excise the excess upper and central abdominal skin for a tighter result from her massive weight loss, but the patient did not want the associated midline abdominal scarring. As part of her procedure we also performed additional VASER liposuction to her flanks and back. This complex procedure was performed as a single-stage with an over-night stay in the hospital, and the patient’s recovery was very smooth.
