This 52-year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting post-bariatric body contouring surgery. She had undergone a laparoscopic gastric sleeve procedure 5 years before, and had lost 70 pounds. She was 275 pounds and now weighed 205. She is 5’3”. Her primary concern was the excess skin and fat that remained around her abdomen, torso, outer thighs, and buttocks. We discussed the Around-the-World Abdominoplasty, which combines an extended abdominoplasty (tummy tuck) with full plication of her diastasis recti, and a posterior lower body lift (butt lift). The posterior lower body lift also includes an outer thigh lift with liposuction to the outer thighs. For her abdominoplasty, we did discuss a possible Fleur-de-Lys Abdominoplasty to excise the excess upper and central abdominal skin for a tighter result from her massive weight loss, but the patient did not want the associated midline abdominal scarring. CASE 692: “Around-the-world” Tummy Tuck with Posterior Body Lift, Lipo to Outer Thighs and Flanks
Age: 48 | Height: 5’3″ | Weight: 176

This 48 year-old woman from the Brownsville, Texas area presented to my plastic surgery clinic for total body sculpting. She is 5’3″ tall and weighed 176 pounds.

We started with an extended tummy tuck with liposuction to her flanks, back, and outer thighs. As part of this first surgery, we also performed a breast augmentation with a donut lift (See Breast Augmentation with Lift photo gallery). The patient was very pleased with her initial results, and was very compliant with her diet and exercise program.

One year later, she returned complaining of worsening cellulite and sagging around her butt and thighs as she continued to loose weight. We discussed a full posterior body lift to remove all the excess skin from her outer thighs and butt to maximize her gluteal contour enhancement. In addition, we performed more liposuction to her outer thighs and laser liposuction to her inner thighs. As part of the procedure, we also performed a scar revision of her prior tummy tuck scar to optimize the circumferential scar. Her weight is down to 167 pounds. Her results are shown only 2 months post-op and she loves her new look after her cosmetic surgery.

This 57 year-old woman from the Corpus Christi, Texas area presented to my cosmetic surgery clinic to fix her botched stomach. She had undergone SmartLipo laser liposuction in Austin, Texas and she felt deformed (note wrinkled and burned tissues in pre-op images from this lipo procedure). Tummy Tuck with Liposuction Flanks
This 32 year-old woman from the McAllen, Texas area presented to my plastic surgery clinic requesting a Mommy Makeover. She is 5’4” tall and weighed 124 pounds. Her primary concern was the loose skin and stretch marks throughout the lower abdomen. She had a prominent diastasis recti and an umbilical hernia, which caused her discomfort. Although she had loose skin and stretch marks throughout her lower abdomen, she was very thin and had very little loose skin in the upper abdomen. As part of her mommy makeover, we performed a tummy tuck with liposuction to her flanks and an inframammary breast augmentation with silicone gel implants. We aspirated 200ml of fat from her right flank, 250ml of fat from the left flank, and 225ml from the abdomen and waist. Because of the lack of skin from the upper abdomen, we used an advanced closure technique to keep her tummy tuck scar low, under minimal tension, and also repaired her umbilical hernia. Her results are shown at 3 months. Her weight is down to 118 pounds, and she loves her cosmetic surgery results.
